A 7 year old intact female dog was presented with a history of swelling in the left inguinal region since one year and increasing in size since one month. Clinical examination revealed animal was dull, lethargic and dehydrated with a large soft to firm swelling observed in the left inguinal region which was irreducible. Ultrasonography findings revealed hypoechoic pockets of uterine horns with thickened uterine walls suggestive of pyometra. Haemato-biochemical parameters showed leukocytosis, severe anaemia, and marked thrombocytopenia suggestive of systemic inflammatory response and toxaemia associated with pyometra. Based on these findings the dog was tentatively diagnosed with left sided inguinal hernia containing pyometra uterus with pyometra. Animal was anaesthetized using atropine sulphate, Xylazine hydrochloride and thiopental sodium. Herniorrhaphy along with panhysterectomy was performed. No post-operative complications were observed and animal recovered uneventfully.
